Fireclay Tile Wellness Program

Fireclay Wellness Program:
All Teammates enrolled in a Fireclay medical plan begin at the “Starting At” rate. Completing wellness activities reduces your premiums throughout the year. All credits are retroactive, meaning activities completed in your first 3 months of 2026 (or within 3 months of hire) apply back to January deductions. Each wellness activity completed by a Teammate earns a monthly credit. When a covered spouse or domestic partner completes the same activity, they earn an additional credit of the same amount (The Internal Benefits 1:1 credit applies to teammates only.)
2026 Wellness Activities

1. Biometric Screening — $60/month
Complete within the first 3 months of 2026 (or within 3 months of hire). Fireclay pays for the screening (for teammates and covered spouses/partners). Onsite screenings in Aromas and Spokane. Distributed teammates may visit a local lab at no cost.

2. Annual Preventive or Primary Care Visit — $60/month
Complete your annual physical within the first 3 months of 2026 (or within 3 months of hire). If you completed your 2025 visit late in the year, the People Team can help align your timing.

3. Internal Benefits 1:1 — $50/month
Complete within your first 3 months of 2026 or within 3 months of hire. Helps you understand your plan, optimize coverage decisions, and navigate care.

4. Non-Smoker Attestation — $80/month
Completed during Open Enrollment or new hire onboarding. If you stop smoking during the year, you may still earn this credit.

For all teammates who have opted out of our Medical Insurance plan, we're thrilled to offer the continued opportunity to participate in our Wellness Stipend Program for 2026—a simple way to invest in your health and earn rewards! You can complete up to two distinct Wellness Incentives, each earning you a valuable stipend. This benefit is exclusively for teammates (not spouses) and once completed, the accrued stipend rewards are processed and paid out to you in a lump sum at the end of each quarter.

Note: The Wellness Benefit will be processed at the end of every quarter by payroll.

Voluntary Worksite Benefits

For this plan year, Fireclay Tile will offer a selection of Voluntary Benefits through Guardian. They are a cost-effective solution to help offset out-of-pocket medical expenses. These benefits can help fill the gaps created by high-deductibles and rising co-pays, providing the financial security they need and deserve. Plan options and costs will be outlined in Rippling.
